Picking up immediately where the second film left off, the survivors in the monster-infested town are rescued by a mysterious prophet who claims to have the power to control the ravenous creatures. As they attempt to reach the safety of a nearby city through a series of underground sewers, the group realizes that the monsters are evolving and becoming even more lethal. With the body count rising and the "prophet's" motives becoming questionable, the remaining survivors must engage in one final, absurdly violent stand against the beastly horde in hopes of reaching a "happy finish" that is anything but certain.
