Bruno, a former doctor, has lost his medical license and now scrapes by on the fringes of the law. In the shadows of Cologne's underworld, he treats those who prefer not to seek medical help through official channels, including prostitutes and petty criminals. His daily life is a balancing act between moral principles and sheer existential fear. But when lawyer Kreber offers him 50,000 euros to treat a mafia boss suffering from leukemia, he senses an opportunity to free himself from his precarious situation. The treatment turns out to be a nightmare. Bruno realizes that something is wrong with the antibody serum sold to him by a pharmacist friend. He confronts him, but doesn't get his money back and has to find the serum elsewhere. Procuring the life-saving antibodies becomes a risky game with unreliable dealers who cheat and blackmail him. At the same time, the situation escalates when Bruno's brother-in-law Giuli, a brutal henchman for the Italian mafia, discovers that his own brother-in-law wants to keep their hated adversary alive. The tensions between the men, who were once family, erupt into a spiral of violence and betrayal. Bruno, who only wanted to heal, suddenly finds himself forced to fight for his own life. The Cologne underworld becomes the scene of a merciless struggle for survival.
